The Secretin-cholecystokinin test is a diagnostic procedure used to assess the function of the pancreas and the gallbladder. This test evaluates the response of these organs to the hormones secretin and cholecystokinin (CCK), which are naturally occurring hormones involved in the digestive process.
Procedure
The test involves the administration of secretin and cholecystokinin intravenously. Secretin is a hormone that stimulates the pancreas to release a bicarbonate-rich fluid, while cholecystokinin stimulates the gallbladder to contract and release bile. During the test, a tube is inserted through the patient's nose, down the esophagus, and into the duodenum to collect pancreatic secretions and measure their volume and bicarbonate concentration.
Secretin
Secretin is a hormone produced by the S cells of the duodenum in response to acidic chyme entering from the stomach. It primarily stimulates the pancreas to secrete a bicarbonate-rich fluid that neutralizes the acidity of the chyme, providing an optimal environment for the function of digestive enzymes.
Cholecystokinin
Cholecystokinin is a hormone secreted by the I cells of the duodenum and jejunum. It stimulates the gallbladder to contract and release stored bile into the duodenum, aiding in the digestion and absorption of fats. CCK also stimulates the pancreas to secrete digestive enzymes.
Clinical significance
The Secretin-cholecystokinin test is used to diagnose conditions such as chronic pancreatitis, pancreatic insufficiency, and other disorders affecting the pancreas and biliary system. Abnormal results may indicate a dysfunction in the secretion of pancreatic enzymes or bicarbonate, or an impaired response of the gallbladder.
