Pivot-shift test

The Pivot-shift test (pronunciation: /ˈpɪvət ʃɪft tɛst/) is a clinical examination procedure performed by medical professionals to assess the stability of the knee joint and diagnose anterior cruciate ligament (ACL) injuries.

Etymology

The term "Pivot-shift test" is derived from the action performed during the test. The word "pivot" refers to the rotational movement, and "shift" refers to the sudden give way or subluxation of the knee joint that occurs if the ACL is damaged.

Procedure

The Pivot-shift test is performed with the patient lying supine. The examiner holds the patient's heel with one hand and the knee with the other. The knee is flexed to 30 degrees and the foot is internally rotated. A valgus stress is applied to the knee while it is slowly extended. A positive test is indicated by a sudden shift or "give way" of the knee joint as it nears full extension, suggesting an ACL injury.
