Ego-dystonic sexual orientation is a term used in the field of psychology and psychiatry to describe a condition where an individual's sexual orientation is at odds with their ideal self-image, causing psychological distress. This condition is often associated with homosexuality, but it can apply to any sexual orientation that is incongruent with an individual's self-perception.

Ego-dystonic sexual orientation is defined by the Diagnostic and Statistical Manual of Mental Disorders (DSM) as a significant psychological distress about one’s sexual orientation. It is characterized by a desire to change one's sexual orientation due to a conflict between one's physical and emotional feelings and one's self-concept.

History[edit]

The concept of ego-dystonic sexual orientation was first introduced in the DSM-III in 1980. It was a controversial addition, as it was seen by some as pathologizing homosexuality. In the DSM-III-R, published in 1987, the diagnosis was limited to those who were distressed by their sexual orientation, not those who were comfortable with it. In the DSM-5, published in 2013, the diagnosis was removed entirely, reflecting a shift in societal attitudes towards homosexuality and other non-heteronormative sexual orientations.

Treatment[edit]

Treatment for ego-dystonic sexual orientation typically involves psychotherapy or cognitive behavioral therapy (CBT). The goal of treatment is not to change the individual's sexual orientation, but to help them come to terms with it and reduce the distress it causes. This can involve exploring the individual's beliefs and attitudes towards their sexual orientation, and working to change any negative beliefs they may hold.

Controversy[edit]

The inclusion of ego-dystonic sexual orientation in the DSM has been a source of controversy. Critics argue that it pathologizes homosexuality and other non-heteronormative sexual orientations, and that it can be used to justify conversion therapy, a practice widely considered to be harmful and unethical.
