Off-Pump Coronary Artery Bypass (OPCAB) is a type of coronary artery bypass grafting (CABG) surgery performed without the use of a cardiopulmonary bypass (CPB) machine. This technique is also known as "beating heart" surgery because the heart continues to beat during the procedure.
History[edit]
The development of OPCAB began in the late 20th century as surgeons sought alternatives to traditional CABG, which requires stopping the heart and using a CPB machine. The goal was to reduce complications associated with CPB, such as systemic inflammatory response syndrome and neurological complications.
Procedure[edit]
During OPCAB, the surgeon stabilizes the area of the heart where the bypass is to be performed using specialized equipment. This allows the surgeon to sew the grafts onto the coronary arteries while the heart is still beating. The procedure typically involves the use of arterial grafts such as the internal thoracic artery or saphenous vein grafts.

OPCAB has several potential advantages over traditional CABG, including:

Disadvantages[edit]

Studies have shown that OPCAB can lead to outcomes comparable to traditional CABG in terms of graft patency and long-term survival. However, the success of the procedure is highly dependent on the skill and experience of the surgical team.
