Pulpotomy
Pulpotomy (pronunciation: /pʌlˈpɒtəmi/) is a dental procedure that involves the removal of a portion of the pulp, including the diseased aspect, with the intent of maintaining the vitality of the remaining pulpal tissue by means of a therapeutic dressing.
Etymology
The term "pulpotomy" is derived from the Latin word "pulpa" meaning flesh and the Greek word "tome" meaning cutting.
Procedure
A pulpotomy is often performed to relieve pain and swelling in cases of pulpitis. It is also used as a preventive measure to avoid pulp necrosis after exposure of the pulp during tooth preparation. The procedure involves the removal of the coronal pulp with a clean, sharp dissection under local anesthesia. The remaining pulp is then treated with a medicament such as formocresol, ferric sulfate, or mineral trioxide aggregate to preserve the remaining pulp and prevent bacterial invasion.
Indications
Pulpotomy is indicated in cases of symptomatic irreversible pulpitis as a means of pain relief and to maintain the vitality of the remaining pulp tissue. It is also indicated in cases of pulp exposure during tooth preparation for restorative procedures, especially in primary teeth.
Contraindications
Contraindications for pulpotomy include teeth with necrotic pulp, teeth with periapical pathology, and teeth with irreversible pulpitis that is symptomatic.
