Electronic Health Record (EHR)
Electronic Health Record (EHR), pronounced /ɪˈlɛktrɒnɪk hɛlθ rɪˈkɔːrd/, is a digital version of a patient's paper chart. EHRs are real-time, patient-centered records that make information available instantly and securely to authorized users.
Etymology
The term "Electronic Health Record" comes from the combination of the words "electronic", referring to an item or system that operates using electricity, "health", referring to the state of being free from illness or injury, and "record", referring to a thing constituting a piece of evidence about the past, especially an account kept in writing or some other permanent form.
Definition
An EHR contains the medical and treatment histories of patients. In addition to standard medical data, and patient demographics, EHRs can include a range of data, comprehensive enough to provide a complete picture of a patient's health.
