Continuity of Care Record (CCR)
Continuity of Care Record (CCR) is a health record standard specification developed by the American Society for Testing and Materials (ASTM), the Massachusetts Medical Society (MMS), the Healthcare Information and Management Systems Society (HIMSS), the American Academy of Family Physicians (AAFP), and the American Medical Association (AMA). The CCR is designed to foster and improve continuity of care, increase patient safety, and ensure a minimum standard of secure health information transport.

It provides a means for one healthcare practitioner, system, or setting to aggregate all of the pertinent data about a patient and forward it to another practitioner, system, or setting to support the continuity of care.
