Electronic Medical Records

Electronic Medical Records (EMR), pronounced /ɪˈlɛktrɒnɪk ˈmɛdɪkəl ˈrɛkɔːdz/, are digital versions of the paper charts in clinician offices, hospitals, and other healthcare facilities. They contain notes and information collected by and for the healthcare providers, and are primarily used to track the care given to patients.

Etymology

The term "Electronic Medical Records" is derived from the combination of the words "electronic", referring to its digital nature, "medical", indicating its use in the healthcare sector, and "records", signifying the documentation of health information.
