Overview

Sotagliflozin is an oral medication used in the management of diabetes mellitus. It is a dual sodium-glucose co-transporter (SGLT) inhibitor, targeting both SGLT1 and SGLT2. This dual inhibition mechanism allows sotagliflozin to reduce blood glucose levels by preventing glucose reabsorption in the kidneys and the intestines.

Mechanism of Action

Sotagliflozin works by inhibiting the SGLT1 and SGLT2 proteins. SGLT1 is primarily located in the small intestine, where it facilitates the absorption of glucose from the diet. By inhibiting SGLT1, sotagliflozin reduces postprandial glucose levels. SGLT2 is found in the proximal tubule of the kidney and is responsible for reabsorbing glucose from the glomerular filtrate. Inhibition of SGLT2 leads to increased glucose excretion in the urine, thereby lowering blood glucose levels.

Clinical Use

Sotagliflozin is primarily used in the treatment of type 1 diabetes and type 2 diabetes. It is often prescribed as an adjunct to insulin therapy in type 1 diabetes to improve glycemic control. In type 2 diabetes, it can be used in combination with other antidiabetic agents to enhance glucose management.

Side Effects

Common side effects of sotagliflozin include urinary tract infections, genital mycotic infections, and increased urination. Due to its mechanism of action, there is also a risk of ketoacidosis, particularly in patients with type 1 diabetes. Patients are advised to monitor for symptoms of ketoacidosis and to maintain adequate hydration.
