OPQRST is a mnemonic used by medical professionals to accurately discern reasons for a patient's symptoms and history in the event of an acute illness. It is especially useful in the assessment of pain.
Components of OPQRST[edit]
Onset[edit]
- Onset refers to the beginning of the pain or symptoms. Questions to ask include: "When did the pain start?" and "Was it sudden or gradual?"
Provocation/Palliation[edit]
- Provocation or Palliation involves identifying factors that exacerbate or alleviate the pain. Questions include: "What makes the pain better or worse?" and "Are there any activities or positions that affect the pain?"
Quality[edit]
- Quality describes the nature of the pain. Patients may be asked: "Can you describe the pain?" Common descriptors include sharp, dull, burning, or throbbing.
Region/Radiation[edit]
- Region and Radiation pertain to the location of the pain and whether it spreads to other areas. Questions include: "Where is the pain located?" and "Does the pain radiate anywhere else?"
Severity[edit]
- Severity is assessed to determine the intensity of the pain. Patients are often asked to rate their pain on a scale from 0 to 10, with 0 being no pain and 10 being the worst pain imaginable.
Time[edit]
- Time refers to the duration and frequency of the pain. Questions include: "How long have you had the pain?" and "Is it constant or does it come and go?"

The OPQRST mnemonic is a vital tool in emergency medicine and primary care settings. It helps healthcare providers gather comprehensive information about a patient's condition, leading to more accurate diagnosis and treatment plans.
