Disease Management (pronunciation: dih-zeez man-ij-muhnt) is a system of coordinated healthcare interventions and communications for populations with conditions in which patient self-care efforts are significant.

Etymology

The term "Disease Management" is derived from the English words "disease" meaning a disorder of structure or function in a human, and "management" meaning the process of dealing with or controlling things or people.

Definition

Disease Management programs are designed to improve the health of persons with specific chronic conditions and reduce healthcare service use and costs associated with avoidable complications, such as emergency room visits and hospitalizations.

Disease Management programs typically include several components, such as patient self-management education, provider use of evidence-based practice guidelines, case management, and care coordination, among others.
